The Tri-State Community Health Center Pharmacy operates as a 340B-participating, HRSA-funded Federally Qualified Health Center (FQHC) pharmacy, designed to expand access to affordable medications while supporting continuity of care for both Tri-State patients and the surrounding community.
Pharmacy Benefits
Tri State provides wellness exams to patients of all ages. We provide the following services to meet your healthcare needs:

On-site prescription dispensing to reduce transportation and access barriers

Patient education and Medication Therapy Management (MTM)

Care coordination with Tri-State’s primary care, women’s health, pediatric, dental, mental health, and substance use disorder programs

Enhanced medication support for chronic disease management and Medication-Assisted Treatment (MAT) programs

In select and medically appropriate circumstances, the pharmacy also offers prescription delivery services to support patients with unique access, mobility, or clinical needs.
Through this integrated pharmacy model, Tri-State Community Health Center Pharmacy is committed to improving medication access, adherence, and health outcomes while continuing to serve as a trusted pharmacy resource for both our patients and the broader community.
Medications & Hours
The pharmacy utilizes both 340B and non-340B (regular) inventory, allowing us to provide:

​Discounted 340B medications for eligible Tri-State Community Health Center patients

Full prescription services using regular (non-340B) inventory for patients who are not 340B-eligible and for members of the general public

Monday and Thursday 09:00 AM -5:00PM
Tuesday and Thursday 09:00AM-7:00 PM
Friday 09:00 -01:00 PM
(Daily: Monday-Thursday Closed 12:00pm-12:30pm for Lunch)
