Tri State Community Health Center is a Federal Qualified Health Center (FQHC), Federal Public Health Service (PHS) deemed status, and receives Federal HHS funding under Section 330 of the PHS Act. Tri state Community Health Center provides affordable and high quality Medical, Dental, Behavioral Health, Women Health (OBGYN), and Case Management services, and affordable 340B Pharmacy program to the resident of Memphis and Shelby County.

Monday and Thursday

9:00AM – 5:00PM

Tuesday and Wednesday

9:00AM – 7:00PM

Friday

9:00AM – 1:00PM
